Navigating the support systems for your needs can feel like an uphill climb. Our experienced case managers are experts at finding the supports you need and building collaboration between them.


A diagnosis does not define a person. Our case managers walk alongside the people they serve with an empathetic ear and deep knowledge of mental health concerns.
Everyone needs help sometimes and seeking help is a sign of strength and resiliency. People with mental illness often need a wide network of services and supports to improve functioning and overall well-being.
Every person and family situation are unique, which is why we never use a one-size-fits-all approach. Case managers work collaboratively to develop a care plan, create pathways to access needed services, and coordinate care across multiple providers. Case managers work in partnership to build bridges to mental health care, social connections, educational opportunities, vocational training and opportunities, recreational activities, and transportation. Our compassionate case managers advocate on behalf of the people we serve’s needs and offer a helping hand and expert guidance to help people of all ages.
Depending on a person’s unique needs, case management includes:
Assessment
Care planning
Advocacy
Housing assistance or coordination
Monitoring and support
Crisis evaluation and assistance
Connection to community wellness and social activities
Our case managers meet with the people they serve at least once per month for a minimum of one hour. In times of stress or crisis, case managers may meet with individuals and families more often, depending on the individual’s and/or family’s unique needs. Our case managers will also reach out to individuals and families by phone weekly and connect with other service providers as needed. Case Managers are mental health practitioners under the supervision of a licensed mental health professional.
